CMMC’s Cardiovascular Disease & Diabetes Prevention Program is a group program supporting participants to incorporate healthy eating and physical activity into everyday life to help prevent Cardiovascular Disease and Diabetes.

The CMMC Cardiovascular Disease & Diabetes Prevention Program is an affordable, one-year program.  For the first 16 weeks, group sessions will be held once a week.  After the initial 16 sessions, classes will be held monthly.  A daily log of exercise and food intake will be expected of all participants.

 

Who Qualifies for the Program?
To qualify, you must have a Body Mass Index (BMI) of 25 or greater, along with ONE of the following:
  • Have a diagnosis of Pre-Diabetes
  • History of Gestational Diabetes
  • Given birth to a baby weighing 9 lbs or more
  • Diagnosis of, or on medication for, High Blood Pressure
  • Diagnosis of, or on medication for, High Cholesterol
   *A medical clearance form from your physician will be required.
